New Patient Registration
COVID update - We are happy for you to scan in your forms and send to our generic email Fairbrook.firstname.lastname@example.org or return into our postbox outside. This will reduce the footfall into the surgery.
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so it would help if you could provide proof by way of a recent utility bill or council tax bill.
Medical treatment is available from the date of registration. Please contact reception for further information.

We know that a number of patients have had difficulty getting appointments here in recent months and we wanted to explain why that is and to let you know that we are working with other organisations to improve this situation.
Practices like ours are not allowed to stop new patients from registering with us so we must continue to take on local people who want to use our services.
The very considerable pressure on space here is making it difficult to fit in appointments so there is a plan to develop new facilities locally. We will keep you posted on that as things progress.
In the meantime, please do bear with us – our team here are doing their best to accommodate all requests for appointments and will continue to help our patients as much as we can
